*if you don’t have a video of a live performance, perform a monologue or similar to a camera (phones are fine), and submit that!

Game Design:

Please attach or provide a link to your portfolio. If you are interested in programming, please provide any code/scripts you have written.

Music Performance:

Outline of your music performance background
Video submission (upload) of you performing in your chosen area (vocalist, guitar, bass or drums)*
